We begin with a free consultation. During this meeting, we listen carefully to the client’s needs, preferences, health conditions, and daily routines. We obtain permission from the client to speak with family members or other support systems to get a full understanding of the situation when and if necessary.
We understand that every situation is different, so we offer flexible options for where consultations and assessments can take place. Our goal is to meet clients and families where they feel most comfortable and to make the process as smooth as possible.
Where Consultations Can Happen
In the Client’s Natural Environment: This is our preferred option when possible. It gives us a clear view of the client’s living environment, which helps us assess safety risks and plan appropriate care.

Care Facilities: If a client is currently in a hospital, rehabilitation center, nursing home, or assisted living facility, we can meet there to begin planning for discharge or transitional care at home.

Virtual Consultations: For families who live out of state or when an in-person visit isn’t practical, we offer secure video consultations. These sessions still allow for detailed conversations about care needs and preferences.
